Smart Forms are sent via the direct message pathway, so you'll need to have both an email address and a mobile number for the intended recipient. 

To send a Smart Form:

Click on Create -> Form/Assessment

Choose which form you'd like to send and click on the icon at the end of the row under the Smart form heading (NB. Do not open the form and then send it to the client as this will send it in a non editable state) :



Select a recipient from parties linked to your active client:



Enter a message, choose an expiry date and click Send:



You'll then see confirmation that your message has been sent.
