How to create a copy of a custom assessment form for editing
To save you time, you have the option to create a duplicate copy of a custom form, which allows you to then make changes to a separate version while maintaining the integrity and layout of your existing form.
For example, you might already have an Initial Appointment form set up and you'd now like to create a Review Appointment form, which contains similar fields but is less comprehensive.
You can create a duplicate of the existing Initial Assessment form, then you can edit the duplicate copy of the form to rename it and change the fields. The duplicate form will then be saved as a separate entry under your forms and assessments, and be available for use either as a smart form or in clinic with a patient.
Go to Main Menu -> Settings -> Assessments
Click on the 3 dots at the end of the row containing the form you'd like to copy and choose Duplicate
You'll then be asked to name the new assessment form. By default the form will be titled [Original name] copy but you can change this. When you are happy with the name, click Save and you will see the new form added to your Assessment Collection.
Forms are listed alphabetically
To make changes to this duplicate form, again click on the 3 dots at the end of row and choose Edit
You can then edit the title and content and rework it into your new form
Once you have finished, click Publish and Exit
Head to Create -> Assessment and the new form will be there ready for use!
